General Data Protection Regulation (GDPR)

General Data Protection Regulations for all businesses come into force on 25th May 2018 to protect personnal data businesses may come into contact with in order to eliminate unregulated, unsafe and unsolicted use of personnal data

As a provider of Medical services Holistic Health and Footcare are required to record details of all our patients/clients we treat,this includes medical information and medical history, along with demographic details.

We as a clinic are required by law to document what personnal information we keep for all our patients/clients.This information is as follows:

• Medical notes will be taken in line with mandatory requirements, including contact details and medical history

• Patien/clientt notes will be stored for a period of no less than 7 years (depending on the patients’ age and ability to consent). They will be destroyed in line with the relevant legislation after this period. Your Right to be Forgotten cannot overide the requirements for medical notes to be held for the above mentioned retention period.

• Paper notes will be stored in a secure, locked filing cabinet. Access to the notes will be by Michelle Scott and Amanda Smiles only.

• Patient/clients names are used in the paper diary; other identifying details will not be included.

• Patient/clients names, addresses, phone numbers are stored on Holistic health and footcares mobile phone, only to be used By Michelle Scott and Amanda Smiles Please inform the us if you do not wish to have your details stored in this manner.

• Patients/clients may be contacted by phone to confirm appointments. Please let us know if you do not wish to be contacted in this way.

• Patients/clients can request to be contacted by text message or email if they prefer this to phone.

• Patients/clients may request copies of the data stored by Holistic Health and Footcare for an admin fee

• Access to patient note may be requested by the governing body for audit purposes; any data will be treated with strict confidentiality and will not be made public. Patients should inform us if they wish to prevent their records from being used for clinical audit purposes.

• Patients may withdraw consent in the future regarding the way their data is stored and used.

• Michelle Scott and Amanda Smiles are both members of HCPC and other relevent professional medical bodies whom require high clinical standards with regards to patient confidentialityand the handling of personnal data.

3rd Party, information sharing and marketing.

• Holistic Health and Footcare do not use your information for anything other than to contact you regarding an appointment, query, or discuss matters relating to your appointment/treatment/therapy and where required to liase with other medical professionals to assist in your care, for example your GP

• We will not ever pass your information on to any 3rd party.

• We will not use your personal information for marketing purposes

Facebook, Email and other Social Media

• Any information given to Holistic Health and footcare via the message service on our Facebook page or email address are only accessible by Michelle Scott and Amanda Smiles. You may request that once we have written your details in your medical records we delete any message which contains your personal information.

Text and Phone messages

• Any information you have provided via text, or phone message is accessible only by Michelle Scott and Amanda Page

• These messages are deleted after we have recorded your details in your medical files.

If you are not happy for this information to be retained we unfortunatley will be unable to provide you with treatment.
